The Occupational Therapist works within a multidisciplinary team setting to provide therapy services to patients in medically complex and slow-paced rehabilitation programs for adult and geriatric populations. They perform assessments and develops and implements individualized therapeutic programs to enhance and maximize patients’ functional abilities. He/she participates in discharge planning to help ensure that patients graduate to the most appropriate living option, whether that be independent living, assisted living, or institutional care.
Pay Rate: CA $35.59/hr - CA$41.88/hr (commensurate with experience)

QUALIFICATIONS:
Clinical Master’s Degree in Occupational Therapy
Current registration with the College of Occupational Therapists of Ontario
ADP Authorizer certification and experience in geriatric rehab would be considered assets

GENERAL ACCOUNTABILITIES:
Assesses each patient’s functional abilities at time of admission as they relate to activities of daily living, taking into consideration cognitive/perceptual status, mobility, requirements for adaptive devices, leisure goals, and social and environmental factors.
Identifies and prioritizes patient needs based on assessment and as indicated by the patient, family and other health care team members.
Develops and implements individualized treatment plans based on identified patient needs including preservation and improvement of mobility and range of motion, with safeguards to prevent injury or decline in ability.
Recommends adaptive equipment, exercise regimes, routines and other activities to enhance independence with respect to activities of daily living, community life skills, mobility and transfers. Fabricate or modify adaptive equipment as necessary based on the needs of the patient.
Monitors patient progress on an ongoing basis as it relates to established patient goals and discharge plan modifies treatment plans accordingly. Complete regular progress evaluations as required.
Supervises, directs, and evaluates the activities of Rehabilitation Assistants.
Completes all relevant forms, applications and other documentation in accordance with professional standards and organizational requirements.
Works collaboration with the patient, family/SDM/POA and the inter-professional team to plan and implement an appropriate discharge timeline, destination and post-discharge follow-up and care.
Participates in care conferences, team rounds and other mechanisms to support communication within the interprofessional team.
Provides patient /family/SDM/POA with information about home programs and education as necessary in preparation for discharge and/or recommend follow-up via other resources available in the community.
Participates in maintaining the care environment including equipment and other material resources.
Assesses patient safety issues and concerns, and actively participates in improvement activities.
Assists other members of the health care team in the delivery of patient care as required.
Supports the knowledge and skill development of other staff and formal learners.
Participates in the identification of clinical or operational improvement opportunities and assist in performance improvement activities.
Performs other related duties related to the work described herein.
ADDITIONAL RESPONSIBILITIES:
Recommending adaptive equipment, exercise regimes and other activities to enhance independence with respect to activities of daily living, mobility, and transfers.
Modifying adaptive equipment based on patient needs.
Providing education to patients and families and advising on post-discharge services.
